Gingival recession is characterized by the apical migration of the gingival margin, resulting in root surface exposure and potentially leading to aesthetic impairment, dentin hypersensitivity, and increased susceptibility to root caries. Several mucogingival techniques have been proposed for root coverage, with the coronally advanced flap associated with a subepithelial connective tissue graft—particularly the technique described by Zucchelli and De Sanctis—standing out due to its predictability and favorable outcomes. The aim of this study was to report a clinical case of multiple gingival recessions classified as Miller Class III in a 41-year-old male patient treated using this surgical approach. The procedure involved the elevation of a full-thickness flap, harvesting of a connective tissue graft from the palatal region, and placement over the exposed root surfaces, followed by coronal advancement of the flap. The donor site was protected using flowable resin. Postoperative follow-up demonstrated satisfactory healing, absence of complications, and significant improvement in dentin hypersensitivity and aesthetic conditions. It can be concluded that the employed technique is effective, predictable, and safe for the treatment of multiple gingival recessions, providing satisfactory functional and aesthetic outcomes.
