Abstract

Objective: To critically analyze the available scientific evidence on restorative strategies for masking discolored dental substrates using direct composite resin restorations, focusing on the factors that influence esthetic predictability. Materials and Methods: A narrative literature review was conducted through searches in the PubMed/MEDLINE, Scopus, and Web of Science databases, including studies published between 2016 and 2026. Laboratory studies, clinical studies, and review articles addressing substrate influence, the optical properties of composite resins, masking ability, the use of opaquers, layering techniques, and the color stability of restorative materials were included. Results: The available evidence demonstrated that masking discolored dental substrates is a multifactorial process influenced by the interaction between the degree of substrate discoloration, the optical properties of restorative materials, restoration thickness, and the restorative technique employed. Materials with higher opacity, combined with appropriate restoration thickness, layering techniques, and the selective use of opaquers, provided more predictable esthetic outcomes, particularly in moderately and severely discolored substrates. Conclusion: Successful masking of discolored teeth depends on the careful selection of restorative materials and appropriate treatment planning, contributing to more predictable and conservative esthetic outcomes.
