Program of elderly attention (P.A.I.): care process

Abstract

ABSTRACT: The increase of elderly population and a predominance of chronic diseases become a becochallenge to the Brazilian public health system to seek health care models of caring to elderly people who require continuous care, complex and interdisciplinary. The Program of Elderly Attention (PAI) is an extension university project to assist elderly in situations of fragility and high risk of hospitalization and also provide the experience of interdisciplinary experience to academic health courses at the Universidade Regional do Noroeste do Estado do Rio Grande do Sul, Ijuí (Brazil). This study objective is describe the medicine use of elderlys assisted by PAI after hospital discharge, considering the conditions of health problems, family background, the overload / stress of family caregivers and the interaction of the interdisciplinary team with the elderly and the family in the care process.
