Abstract The present work tells a clinical case of a female patient with fistula in tooth 12, which had already undergone two apical surgeries. The deficient filling of the root canal demanded endodontic retreatment, made difficult by the presence of intraradicular post and amalgam retrograde filling. After undergoing new periapical surgery, the case has presented clinical silence for a period of eight years. The x-ray with the evidence of bone repair in the region proves the success of the intervention and the good choice in the option of treatment. It is concluded that the diagnosis and treatment of the presented case was correct, as the patient expresses satisfaction with the maintenance of her tooth, excusing the necessity of an osseointegrated implantat. Maintaining the natural tooth is still the best alternative, since osseointegrated implants, like any other one technique, have particularities and limitations and demand an excellent preparation from the professional, in order not to cause problems when incorrectly indicated.
