Surgical Treatment of Mesiodentes Under Conscious Sedation with Nitrous Oxide

Introduction: Supernumerary teeth are characterized as an excess in the number of teeth the normal series. The mesiodentes, defined by their median location to the upper central incisors, are the type of supernumerary tooth most commonly found in the oral cavity, usually intraosseous and asymptomatic. The presence of these teeth may cause changes in permanent dentition and the surgical approach in the mixed dentition phase shows satisfactory results in spontaneous dental alignment. In pediatric patients who are not psychologically conditioned to receive dental treatment, conscious sedation with nitrous oxide has been shown to be a safe and effective alternative in controlling fear and anxiety. Objective: this study aims to report a surgical procedure for the removal of two mesiodentes in a pediatric patient under local anesthesia associated with nitrous oxide sedation. Case report: 11-year-old female patient required surgical removal of two localized supernumerary teeth in the anterior region of the maxilla. With the evaluation of computed tomography, one can observe the exact position in which both were. The extraction of the dental element 14 was also performed as requested by the orthodontist for further continuity of orthodontic treatment. Due to the surgical complexity and operative time and, considering the patient's age and the possible increase in anxiety levels, conscious sedation was chosen with the aid of nitrous oxide. Conclusion: clinically, supernumerary teeth cause local damage and surgical treatment associated with conscious sedation has proved to be quite safe and effective in the behavioral control besides being able to be implemented in the outpatient routine.
