Analysis of the influence of subepithelial connective tissue graft thickness on root coverage

Abstract

The objective of this review was to evaluate clinical and patient-centered outcomes in relation to the thickness of subepithelial connective tissue grafts (ETCS) used in root coverage surgeries in single and multiple gingival recessions (RG). A literature search was carried out to answer the question (PICO): in patients who received a coronally positioned flap+ETCS in Miller class I and II GRs, does the thickness of the graft have an influence on the outcomes evaluated? The main databases PubMed, Scopus and ISI Web of Knowledge were consulted, including studies from the last 20 years. Those selected were subjected to the inclusion and exclusion criteria and 26 articles were included (16 single/10 multiple RG), 258 excluded after manual search, as they did not present the thickness of the graft used. The studies were divided into thin grafts ≤ 1 mm and thick grafts ≥ 1 mm thick. The average percentage of root coverage (Rec Rad) with thin grafts was 79.53/93.44 (unitary: 4 studies / multiple: 7 studies), with thick grafts 76.18: 11 studies/ 83.12: 3 studies. One study evaluated patient-centered outcomes, thin grafts were considered more aesthetic and had lower morbidity. It is possible to conclude based on current studies that there is no difference in the Rec Rad rate (in single or multiple GR) between thin or thick grafts, however, to avoid overtreatment and using patient-centered outcomes, thin grafts (< 1 mm ) promoted less morbidity at the donor site and did not alter the rate of root coverage.
